Abstract
The Effects of a CPR Coach on Compression Quality in a Critical Care Hospital via Simulation
Monica Anne Padgett, MSN, RN, CDCS
Abstract
High-quality cardiopulmonary resuscitation (CPR) is essential during cardiac arrest. The quality of CPR is determined by several elements, including compression depth, recoil of the chest, compression rate, and chest compression fraction. Cardiac Chest Compression Fraction is the calculated amount of time that the compressor during CPR is on the chest and compressions are occurring. The American Heart Association (2020) recommends a chest compression fraction greater than 80%, a depth of ≥55mm on adults, a rate between 100-120, and 100% recoil of the chest. The project agency uses the term “Code Blue” to initiate resuscitation efforts for cardiac arrest patients. According to the American Heart Association (2020) a CPR coach will coordinate the code team, give feedback regarding chest compressions, and help to minimize the length of pauses during compressions. Coaching may be completed by use of a live coach or a coaching device.
The purpose of this process improvement project was to determine if the chosen agency’s current compression quality, in terms of compression depth, speed, chest recoil, and compression fraction, via simulation using CPR manikins and the Laerdal CPRmeter-2, and compare these numbers to those collected after a CPR coach has been implemented. The primary investigator found that there was a significant increase in rate, depth, recoil, and chest compression fraction. Confidence of those completing compressions was increased as well.
