Abstract

Abstract

Pressure Injuries (PI) are a major safety concern with a potential for immobility, grief, shame, fluid leakage, feeling of isolation and yet 95% of them are avoidable. Clinical team members are responsible for caring for patients to ensure that those admitted with pressure injuries are improving and not worsening. The cost per day to treat PIs is higher than for prevention. In the United States, 60,000 people die each year because of PI complications. The purpose of this project is to: 1) implement evidence-based PI education 2) increase skin assessment and documentation on the avatar within the electronic medical record, 3) increase documentation compliance of Braden scale documentation in the electronic medical record system, 4) provide the PI staging decision tree to the clinical team, and 5) increase the pressure injury knowledge of the clinical staff members on the ventilator unit located in a long-term care nursing home in New York. The increase in pressure injury knowledge, application of the clinical practice guidelines from the National Pressure Injury Advisory Panel, and incorporation of PI staging decision tree shows prevention of worsening of existing pressure injuries.
