Abstract

Bedside shift report is the recommended form of nursing shift-to-shift handoff and has positive effects on nurse-patient communication, but it is difficult to implement and sustain. The organization’s standard for nursing shift-to-shift handoff is bedside report; however, report was not routinely taking place at the bedside. Key opportunities for nurse-patient communication were lost and the organization’s patient experience scores related to nurse-patient communication were well below state and national averages. A pre- and post-test design quality improvement Doctor of Nursing Practice project was conducted using the Plan, Do, Study, Act framework. The objective of the project was to increase the performance of bedside shift report on a pilot unit, utilizing a convenience sample of nurses from the unit (n=36). After surveying nurses to get their perceptions of report, tailored bedside report education was delivered and a bedside report visual aid was distributed. Report was observed pre- and post-intervention. There was a statistically significant increase in the rate of bedside shift report from 14% to 57%, but the rate of bedside report remained below the goal of 90%. The objective of a 10% increase in the pilot unit’s point-of-care patient-experience scores related to nurse-patient communication was not met. Lessons learned during this project include the limitations of voluntary surveys and self-report measures as well as the importance of bedside nurse buy-in, creating sustainable bedside report accountability, and addressing the quality of bedside report. Lessons learned will inform future quality and process improvement projects.
