Abstract

Social Determinants of Health (SDH) are factors that affect a patient’s health outcomes but are not medical measurements, prescriptions, or therapies. Instead, they are external forces or social risks a person faces based upon where they live, work, and play. Screening for these risks allows a medical team more information about a patient and their situation and can contribute to personalized care. Screening patients for these determinants in a community health primary care setting is important and is increasingly being aligned with regulations and reimbursements. The purpose of this Doctor of Nursing Practice quality improvement project was to implement change in clinic processes to increase screening rates, coding, and documentation of SDH in a federally qualified health center. An evidence-based program was presented to demonstrate efficient and retrievable electronic health record methods to support these changes. Two proportion Z test of the data between pre- and post-intervention indicated a significant increase for overall screening rates, documentation of coding for food insecurity, transportation insecurity and housing instability, along with community resource utilization for all four elements: housing, food, financial and transportation. Recommendations for future sustainability of SDH screening education are discussed to ensure patients are routinely assessed in an efficient and effective manner to help support their social needs.
