Abstract
Alcohol use disorder (AUD) remains a significant public health concern in the United States, with disproportionately higher rates in Kentucky. Despite the availability of various treatment modalities, barriers to access and perceptions of treatment effectiveness continue to influence recovery outcomes. This study examines the accessibility and perceived success of alcohol rehabilitation programs in Kentucky through qualitative focus groups with individuals with lived experience of AUD (n = 9). Using an inductive thematic analysis approach, four major themes were identified: individual readiness for recovery, social support and resource navigation, structural barriers within the treatment system, and perceived characteristics of effective treatment. Findings indicate that while personal motivation in an important component of recovery, it is inadequate on its own. Financial constraints, insurance limitations, and distrust of treatment systems emerged as major barriers to care. Contrarily, family support, peer-based community settings, and structured programming positively influenced treatment engagement and success. These results highlight the importance of addressing structural and social determinants in conjunction with individual factors to improve treatment accessibility and outcomes. Implications for public health practice and policy include expanding affordable treatment options, strengthening support systems, and improving consistency and transparency of care. Future research should explore stigma, cultural influences, and regional disparities to further inform policy and intervention efforts aimed at reducing the burden of AUD in Kentucky.
