Book Review

Abstract

The updated fifth edition of Documentation Manual for Occupational Therapy (Gateley, 2024) offers instructors of occupational therapy (OT) and occupational therapy assistant (OTA) students familiar tools and updated content. Clear examples of every section of a SOAP (Subjective, Objective, Assessment and Plan) note as well as steps to reach the composition of the note, such as skilled-problem statements and COAST (client, occupation, assist levels, specific condition and timeline) goals are tools that hold the client-centered and occupation focus clear. Worksheets within the chapters offer formative assessment of understanding of concepts. Specific updates to the fifth edition include expanded content on billing and skilled documentation concerns. The text is closely aligned with the Occupational Therapy Practice Framework: Domain and Process (4th ed.) and other core guidance from the American Occupational Therapy Association.

Biography

Mary K. Thelander, MOT, OTR/L, ATP has been an instructor of fieldwork and graduate students in documentation subjects since 2011 with the University of New Mexico Occupational Therapy Graduate Program. Mary’s clinical practice in community practice, wheelchair clinic and the Veterans Administration offer insight into how documentation can support client’s needs in various settings.

Geneva L. Nolan, MOT, OTR/L has been a fieldwork educator since 2018 and an instructor in a Masters program since 2021. Geneva has clinical practice experience in Inpatient Rehabilitation and Acute Care and also served as fieldwork educator for new graduates in each of those settings.
